Rate evidence, not aspiration

Complete the diagnostic separately before comparing answers. A score is a conversation starter, not an audit finding, accreditation, legal conclusion or public rating.

  • 1 means rarely true; 5 means consistently true.
  • Record evidence for the strongest and weakest rating.
  • Ask whose experience may be missing.

  • Name one practice to stop.
  • Name one practice to protect.
  • Name one process to redesign.
  • Assign an owner, evidence of completion and review date.
